What you'll be doing
As the Youth Health ED Care Navigator, you’ll play a key role in improving healthcare access for marginalised young people who frequently present to Emergency Departments. You’ll help them navigate the health system and connect with primary and community care.
Deliver expert clinical care and case management
Develop frameworks to improve service access
Train and support health professionals in youth care
Build referral pathways and coordinate care across services
Participate in research and quality improvement initiatives

Essential Criteria:
Registration: Current APHRA Registration – registered Nurse/Midwife
Qualification: Post - registration qualifications
Selection Criteria:
Demonstrated understanding of and experience working with young people including young people from marginalised and at risk groups
Demonstrated experience working collaboratively with other service providers including General Practitioners, Community Services, Non-Government Organisations and the ability to work within a multidisciplinary team
Demonstrated understanding of NSW Health initiatives for Youth Health, NSW Government Youth Initiatives and ability to implement the principles of Primary Health Care
Demonstrated high level communication, liaison, negotiation, time management, computer and presentation skills
Knowledge of and/or experience in Acute service settings
Research & evaluation experience, including data collection and data entry
Post- registration qualifications and at least 3 years’ experience working in the clinical area of the specified post graduate qualifications.
