What you'll be doing
Join a dedicated rehab team providing essential social work support to patients and carers in the SMART service. This role blends clinical practice, education, and development.

  • Conducting psychosocial assessments and interventions
  • Collaborating with medical teams on complex cases
  • Engaging in quality improvement and research
  • Educating and support staff and social work students
  • Participating in supervision and professional development

Essential Criteria:

  • Qualification: Degree in Social Work with eligibility for membership of the Australian Association of Social Work (AASW)

Selection Criteria:

  1. Demonstrated understanding of psychosocial issues associated with acute and chronic illness and rehabilitation medicine.
  2. Demonstrated capacity to provide counselling and case management with patients/families with illness adjustment, end of life care and with complex discharge issues.
  3. Demonstrated clinical experience or understanding of the psychosocial impact of grief, loss and bereavement.
  4. Demonstrated understanding of issues associated with working with Culturally and Linguistically Diverse client group.
  5. Demonstrated well-developed time management skills with excellent written and verbal communication, including report writing skills and high-level computer literacy.
  6. Demonstrated ability to work with minimal supervision and as part of a multidisciplinary team including the ability to interact constructively and collaboratively with a diverse range of stakeholders, throughout all organisational levels.
  7. Knowledge of quality assurance and / or clinical practice improvement activities 8. Unrestricted drivers licence and availability to participate in the After-Hours Crisis On Call Social Work Service.
