Remuneration: $32.79 - $35.63 per hour
Hours Per Week: 38

Where you'll be working
Western Sydney Local Health District (WSLHD) is responsible for providing primary and secondary health care for people living in the Auburn, Blacktown, and The Hills Shire, Holroyd and Parramatta Local Government Areas (LGAs) and tertiary care to residents of the Greater Western Region. The purpose of this position is to make timely, safe, appropriate referrals and appointments as identified in a patient care package in consultation with patients, carers and treating clinicians.
Additionally, the role is required to follow up referrals to ensure they are actioned and services are delivered. The position will work closely with the other members of the multidisciplinary team through liaising and advising them of referral outcomes, issues and service provider capacity.

- Registration: Current Enrolled Nurse National Registration APRHA
Selection Criteria:
- Current enrolment with Australian Health Practitioner Regulation Agency as Enrolled Nurse Division 2.
- Proven effective and open communication skills both written and verbal, that build greater cooperation and confidence in care delivery.
- Demonstrated recent and relevant skills and experience in chronic and complex care management
- Experience and working knowledge of Information Technology systems currently used in providing patient care in WSLHD such as CERNER, iPM and Microsoft applications such as Word, Excel and Outlook.
- Demonstrated ability to work within a collaborative team environment with a focus on delivering organised and prioritised care, in order to achieve the most effective and efficient outcomes for the patient.
- Demonstrated ability to ensure patients, carers, and families are well informed, engaged and confident about their care and treatment, within the scope of an Enrolled Nurse.
- Demonstrate critical thinking abilities, prioritisation and escalation, whilst effectively follow directions.
- Understand and practice person centred care. Participate in quality improvement activities. Work in partnership with consumers on improving and evaluating the delivery of services. Ensure timely and accurate reporting of near or actual, incidents or patient safety concerns.
