What you'll be doing
Join the TACP team to support older adults transitioning from hospital to home through client-centred rehabilitation and safe discharge planning. Your responsibilities will also include:

  • Delivering evidence-based physiotherapy care
  • Working closely with a multidisciplinary team
  • Supporting clients to achieve functional goals
  • Promoting safe and timely community discharge
  • Contributing to quality and service improvement

This is a temporary full time position with the possibility of extension, offering an excellent opportunity to further develop your skills in a supportive and collaborative environment.

Essential Criteria:

  • Qualification: Degree in Physiotherapy OR Equivalent 
  • Registration: APHRA Registration  

Selection Criteria:

  1. Degree in Physiotherapy or equivalent, current registration with the Australian Health Practitioner Registration Agency (AHPRA) and current unrestricted NSW Drivers licence.
  2. Demonstrated Physiotherapy experience, including aged care and/or rehabilitation and experience delivering Physiotherapy in the community setting.
  3. Demonstrated computer literacy, high level verbal and written communication and interpersonal skills, including the ability to adapt communication style to suit different audiences.
  4. Demonstrated high level of organisational and time management skills.
  5. Demonstrated commitment to professional development, evidence based practice and continuous improvement.
  6. Demonstrated ability to work both independently and as a member of a multidisciplinary team.
  7. Demonstrated skill in the comprehensive assessment of older adults, and people with physical disabilities and the development of relevant care plans.
  8. Demonstrated ability to analyse problems, set priorities and apply a range of options to develop solutions
