About the opportunity


An exciting opportunity exists for Geriatricians looking to develop a new model of care in a rural context. Western NSW LHD provides care to people living in an area covering about 1/3 of NSW. We are establishing an aged care outreach service for older people with acute illness who are at risk of hospitalisation. Job Share may be considered for this position.

The service will have a network of staff based at multiple locations across Western NSW to provide a hybrid face to face and telehealth outreach service.  You will join a team which includes nurse practitioners, registered nurses, a pharmacist and admin staff.

You will work closely with the Aged Care team, clinical staff in our hospitals and community settings as well as staff in Residential Aged Care homes. Your work will be a mix of outreach into residential aged care, outpatient clinics and acute care consultations.

The service will initially operate between 0800-1700 Monday to Friday with the option to extend hours as the service develops.

We have a partnership with the Geriatric Medicine Service at Sydney Local Health District for outpatient services and collegial support.

Where you'll be working

The Geriatrician will be part of the newly established Western NSW LHD Aged Care Outreach Service (ACOS). This service provides outreach support to older people living in Residential Aged Care Homes (RACH), Multipurpose services (MPS) and in community settings that have an acute illness and are at risk of hospitalisation.

Aged Care Outreach Service – day to day service operations 

Acute care teams including ED, Inpatient and Hospital in the Home teams- provide specialist input for older people from RACH, MPS and community settings requiring acute care 

Residential Aged Care Homes and General Practitioners – consult advice for management of older people and service development

1.       Eligible for registration with the Medical Board of Australia as a Specialist, and/or overseas trained specialist pathways.

2.       Demonstrated ability to work effectively as part of a multidisciplinary team and provide leadership of a team.

3.       Demonstrated effective clinical communication through the use of information technology and written, verbal and interpersonal skills.

4.       Demonstrated commitment to teaching, continuing education, research and quality improvement.

5.       Availability to participate in an on-call roster and to respond on site within a reasonable time.

6.       Fellowship of the applicable College.
