About the role

The Social Worker will be working as part of a multidisciplinary team, and is responsible for providing assessment, case management, referral and follow up for clients seeking assistance with substance use problems. You will be working in a small, dynamic team to provide psychosocial counselling and support to clients with substance use issues. This will require sound knowledge of drug and alcohol clinical information to ensure the client is assessed and case managed appropriately.

You will work Monday to Friday 8:00am to 4:30pm. The role includes providing services virtually and face to face to clients in the community.

The team includes, Social Workers, Health Clinician, Nurses, Addiction Medicine Specialists, Nurse Practitioners and Clinical Lead.

Located within Western NSW Local Health District.
