About the role
Western NSW Local Health District (WNSWLHD) is seeking an experienced and motivated Clinician to lead the implementation of an innovative and much-needed service for older people across the district. This is an exciting opportunity to support the delivery of the Older People's Aftercare Service pilot and the WNSWLHD Older People's Suicide Prevention Pathway, providing clinical leadership and specialist expertise to improve outcomes for older people following a suicidal crisis.

Working within a multidisciplinary team alongside experienced clinicians, Universal Aftercare service providers and other service partners, you will coordinate aftercare support, facilitate care planning and service navigation, and provide consultation and advice to clinicians and service providers. The role will also focus on workforce capability building through education, mentoring and clinical leadership in suicide prevention, risk assessment, safety planning and aftercare practices.

This is a hybrid position combining virtual and face-to-face service delivery across Western NSW. Attractive working hours of 8:30am to 5:00pm, Monday to Friday.

This is a unique opportunity to make a meaningful difference in the lives of older people and contribute to the implementation of a critical suicide prevention initiative across Western NSW.
