You’ll work closely with individuals and families to simplify access to care, strengthen referral pathways and support people to manage their health within their own communities.

About the Role
As a Primary & Community Health Registered Nurse, you will:
- Deliver nursing care through a flexible service delivery model, across community and ambulatory settings
- Build strong, ongoing relationships with patients, carers and community members
- Lead, direct and support other care roles including Registered Nurses, Enrolled Nurses, students and Aboriginal Health Workers
- Contribute to chronic disease prevention, health improvement initiatives and evidence-based hospitalisation prevention strategies
- Work as an integral member of a multidisciplinary care team focused on continuity, quality and culturally safe care

About Lightning Ridge Multi-Purpose Service
Lightning Ridge MPS is a modern, recently upgraded facility delivering a wide range of integrated health services, including:
- Acute and subacute care (including respite and palliative care)
- Emergency services
- Residential aged care
- Mental health and drug & alcohol services
- Primary and community health
- Visiting specialists and allied health services
- 24-hour on-call General Practitioner support
The upgraded facility provides expanded space and amenities to support ageing populations and people living with chronic conditions, ensuring continuity of care close to home.
