About the role
The Aged Care Outreach Service is seeking a dedicated and experienced Registered Nurse to join its multidisciplinary team.
This role is responsible for delivering high‑quality outreach nursing care to residents of Residential Aged Care Homes (RACHs) and older people living in the community. The role is key to providing proactive, person‑centred clinical support designed to enhance quality of life, reduce avoidable hospital presentations, and ensure timely, integrated urgent care for older adults.
