About the role
The specialist clinician, working as part of a multidisciplinary team, is responsible for coordinating the care of clients seeking services within their specialty area and providing expert and quality clinical services in collaboration with consumers, carers and other care providers, using an integrated approach. This position will undertake the role of Assessor for the Single Aged Care Assessment Service and to provide client-end assessment.
The Aged Care Services Division delivers a range of clinical and non-clinical services across the Western NSW Local Health District to support older people in their families, to enable them to remain supported in their home.
The Aged Care teams and programs have a strong commitment to advocacy, restorative care and reablement back to wellness and independence. The position will enjoy cooperation and support within a strong leadership team and can expect significant challenges in a changing aged care environment which needs to respond to a growing patient population and Commonwealth and State health reforms impacting on operations.
