About the role
Ability to coordinate referrals and management of various physical activity programs which may include Falls Prevention, cardiopulmonary rehabilitation and Healthy Lifestyles activities such as walking groups and aqua. Facilitate group exercises programs for community members - complete assessments, document and evaluate outcome measures.
Working Hours: Monday to Friday 8.00am to 4.30pm
This position sits within both the Community Nursing and Physiotherapy teams of Lachlan health service. Clients are often shared / supported between both services. Direct Clinical Supervisor is the Nurse Unit Manager for Community Nursing with the Physiotherapy Manager supervising day to day activities, service programming and education. The team is very motivated and patient focused.
This position works with the similar Parkes position to provide services across both sites. Education, planning and supervision sessions are conducted together. The role is ideal for someone who enjoys being with people and assisting them to make improvements in their lifestyle to remain active and healthy in the community.
