About the role 

Join the Western Virtual Hub & Command Centre – Connecting Care from Hospital to Home.

An exciting opportunity is available to join the Western Virtual Access Hub & Command Centre, a dynamic service dedicated to improving access, continuity and outcomes across the Western NSW Local Health District.

This unique role offers Enrolled Nurses the chance to broaden their scope of practice while playing a central role in connected care pathways across the region. You will be part of a service that truly bridges hospital and community care, supporting patients to receive the right care, in the right place at the right time.

Operating Monday to Friday 8:30am to 5:00pm, you will be responsible for screening, assessing and supporting individuals into community-based and out-of-hospital care programs. A key focus of the role is improving access across the Health District by ensuring timely, efficient and equitable healthcare delivery.

Working closely with a multidisciplinary team, the role provides expert care coordination and case management to facilitate timely, culturally safe and well-coordinated access to services. By guiding patients through their care journey, the role helps prevent unnecessary and unplanned Emergency Department presentations and promote safe, effective out-of-hospital care through the Planned Care for Better Health program.

Your contribution will strengthen continuity of care, improve patient outcomes and ensure individuals across the District are connected with the services and supports they need.
