About the role

An exciting opportunity exists for a dynamic and motivated Diabetes Nurse Educator with a passion for managing Type 1, Type 2 & Gestational Diabetes to join our outpatient Chronic and Complex Care Team.

  • As a CNC2 you will work as a member of a multidisciplinary team to provide patient centred and best practice complex clinical nursing services to clients with Diabetes
  • You will work predominantly in an outpatient community health setting to provide direct patient care and self-management education via face-to-face clinics with individuals and groups and will offer telehealth services and some inpatient services
  • Your role will also include participating in service planning and evaluation; formulating, implementing and updating policies and local operating procedures; developing education resources; guiding practice development; and engaging in quality improvement, education and research activities
  • You will also act as a resource for, and provide clinical leadership and supervision to other staff members & students
  • You will have a full orientation to the service and ongoing support from managers.


The Chronic & Complex Care team is part of the wider primary and community health team. Our dynamic and highly functioning team work to provide multidisciplinary models of care and collaborate with hospital and community health teams and other community-based services to promote health and wellbeing and to address patients’ chronic health needs.
