At Central Coast Local Health District (CCLHD), we are committed to Trusted Care, Better Health for Everyone, and we are looking for a dedicated Registered Nurse (RN) to be an essential part of our Community Nursing service.


About the Opportunity

This is an exciting opportunity for an experienced RN with demonstrated skills in Palliative Care to work within the CCLHD Community Nursing service, providing comprehensive care for the needs of clients, carers and families of loved ones living with a life limiting illness.

The role will provide clinical and consultative support, along with education and advice to the CCLHD Community Nursing Teams. The role will also develop relevant and appropriate care standards and will facilitate formal links with other primary care providers.

In this role you will:

  • Deliver high-quality, evidence-based nursing care in line with professional standards and scope of practice to support patient health and wellbeing goals.
  • Assess, plan, implement and evaluate nursing interventions to achieve positive outcomes for patients and clients in the community.
  • Coordinate care and collaborate effectively with multidisciplinary teams, patients and carers to ensure holistic, patient-centred service delivery.
  • Lead, delegate and supervise Enrolled Nurses and Assistants in Nursing to maintain safe, efficient and high-quality care.
  • Maintain accurate and timely clinical documentation and actively participate in quality improvement and professional development initiatives.

About You

We are looking for someone who has:

  • Current Authority to Practice as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA) (Division 1).
  • Strong communication skills, including well-developed interpersonal and negotiation abilities.
  • Recent nursing experience in Palliative Care or a demonstrated interest in Community Nursing.
  • Excellent time management skills, with the ability to work both independently and collaboratively within a team.
  • Proven ability to collaborate with multidisciplinary health professionals to support effective case management and care planning.
