About the role
We are seeking a highly motivated Aboriginal Health Worker – Transition to Practice to join our skilled and supportive Community and Allied Health team at Cowra and Grenfell. This role is responsible for the provision of high-quality, culturally comprehensive services to support optimal health outcomes for Aboriginal patients, while working towards meeting the requirements for registration with the Australian Health Practitioner Regulation Agency (AHPRA) as an Aboriginal Health Practitioner. The position will provide care across both admitted patients and outpatient settings, ensuring continuity and coordination of care.

The Aboriginal Health Worker at Cowra Health Service provides a crucial link between the Health Service and the community. They play an essential role in establishing and strengthening relationships between Aboriginal patients and clinicians to achieve the best possible health outcomes. The Aboriginal Health Worker delivers culturally appropriate, clinically competent care and health education to patients, and supports staff across the Health Service to deliver culturally safe and responsive services, including participation in health promotion initiatives.
The role works closely with admitted and outpatient services, and networks and liaises with the multidisciplinary team, as well as Government and non-Government service providers, to ensure a coordinated approach to healthcare delivery. The position is operationally accountable to the Executive Team at Cowra Health Service through the Primary and Allied Health Manager. You will be supported by your local team, the broader multidisciplinary team, and the Aboriginal Health Leadership team within the strategic directorate.
