About the role
This is a varied role working closely with another Aboriginal Health Practitioner out in the community at various facilities, as well as within the Diabetes Unit.  You will be providing education for patients and families living with Diabetes including co-ordination of the Wellness Program and Elders exercise group, held twice weekly.

You are an integral member of the multidisciplinary team of Allied Health Professionals which includes Diabetes Educators, Dietitians, Administration Staff, Endocrinologists and Paediatricians, who provide flexible, holistic and culturally sensitive clinical care to Aboriginal and/or Torres Strait Islander peoples.  You’ll be part of shaping the future of health care to your mob.
