About the role
Our single goal is providing a service that builds 'healthier rural people and thriving communities', and in this role, you’ll grow long-term relationships with patients, simplifying referral pathways and access to care to help them manage their health within their own communities.

As an important part of the multidisciplinary care team, you’ll deliver nursing care through a flexible service delivery model, both in community and ambulatory settings and will lead, direct, and supervise other care roles in the team, including nurses, students, and Aboriginal Health Workers (AHW) to support the delivery of quality care.
The successful applicant will demonstrate knowledge and experience in chronic disease prevention, health improvement initiatives and other evidence-based hospitalisation prevention strategies to assist in the overall health and wellbeing of the community of Western NSW. Previous experience working in the Community Health setting and skills in advanced physical assessment, cannulation and wound care would be advantageous.
You will join a small, close-knit team of nursing and Allied Health staff covering Molong, Yeoval, Cudal, Cumnock and Manildra communities. Nursing staff also work in the Ambulatory Care unit at the Molong MPS. You will be well supported, and your dedication will be seen and appreciated. You will be an integral member of the community and you’ll see not only the results of your care, but the appreciation of the community of the role you play in their health.
