About the role
The Aged Care Access Centre (ACAC) is our local health district's hub for intake and allocation of referrals for Aged Care services, Geriatric Medicine, and Out of hospital care home services.
The role of the Intake Officer is to ensure referrals contain sufficient information to link the client to the most appropriate client pathway. Whilst processing referrals you will speak with many different people including clients, family members, hospital and community staff as well as other internal and external stakeholders.
The Access Centre is currently a team of 10 working as a part of the greater Aged Care team consisting of clinical & non-clinical assessors, support staff and leadership team across the local health district.
Working hours are 8:30 to 5:00 Monday to Friday
