Employment Type: Temporary Full Time or Part Time, upto 38 hours per week until April 2027 . Temporary roles are available to visa holders, with contracts issued in accordance with visa conditions. Visa holders will be considered where no suitable Australian citizen or permanent resident is identified. 

Remuneration: $41.22 - $57.88 per hour+ 12% Superannuation + Salary Packaging

The Transitional Aged Care Program (TACP) offers the chance to make a meaningful difference in the lives of older adults by supporting their journey toward reablement, independence and personal goal achievement. As an Registered Nurse, you will play a vital role in delivering nursing care to clients within their own homes in the Macarthur area. Your responsibilities will include completing holistic nursing assessments, wound care, pressure injury assessment & treatment. Client education including but not limited to continence, diabetes, medication management. You will contribute to a unique, client‑focused program while providing high‑quality clinical services that optimise health outcomes for clients and their families. You will also support the smooth operation of the service by completing essential departmental and administrative tasks.

-Enjoys working independently as well as collaboratively within a multidisciplinary team 
-Communicates clearly and confidently

Selection Criteria 
To help us assess your suitability for the role, please tell us as much as you can about yourself in response to the questions below. 

  • Current Authority to Practice as a Registered Nurse with Australian Health Practitioners Regulation Agency (AHPRA)
  • Describe your experience in providing comprehensive nursing assessment, care planning and clinical management for older people with complex health needs
  • Provide an example that demonstrates your ability to work collaboratively with older people, their carers and a multidisciplinary team to support safe transition from hospital to home
  • Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
