This is an exciting opportunity for a Registered Nurse to join the Keeping Well in the Community – Care Navigation and Care Coordination team, supporting clients with chronic and complex health needs to remain well and connected to the right care in the community.
In this role, you will undertake comprehensive assessment, screening, care planning and health outcome monitoring using a predominantly telephone‑based model, working closely with clients, families and carers to identify needs, set goals and support self‑management. You will collaborate with multidisciplinary teams, GPs, acute services and community providers to ensure each client receives coordinated, person‑centred and culturally responsive care. This position plays a key role in reducing avoidable hospital presentations and supporting timely, safe transitions from acute care settings. You will contribute to SWSLHD’s commitment to delivering safe, compassionate and high‑quality care, aligned with the NSW Health Strategic Framework for Integrating Care and the district’s Transforming Your Experience principles.

Selection Criteria
To help us assess your suitability for the role, please tell us as much as you can about yourself in response to the questions below.
- Do you have current Authority to Practice as a Registered Nurse with Australian Health Practitioners Regulation Agency (AHPRA)?
- Demonstrated recent experience as a Registered Nurse providing assessment, planning and care for clients with chronic and/or complex health conditions, including the ability to identify changes in health status and respond appropriately.
- Proven ability to engage and communicate effectively with clients, families/carers and multidisciplinary health providers to deliver safe, compassionate, person centred and culturally responsive care within a telephone based service model.
- Demonstrated experience supporting health literacy, education and self management approaches for clients with complex healthcare and psychosocial needs, and contributing to quality care outcomes.
- Demonstrated effective time management, critical thinking and organisational skills, including the ability to work independently and collaboratively in a fast paced environment while maintaining high quality documentation and professional standards.
Do you have a current Australian C-Class driver's license (including P1 or P2)? if you hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
