Join the Assertive Community Management Team within SWSLHD Drug Health Services, where you will play a vital role in delivering recovery-focused care to individuals impacted by substance use.
- Provide comprehensive drug and alcohol assessments and contribute to integrated, evidence-based care planning
- Actively engage clients with complex psychosocial and substance use disorders, building strong therapeutic relationships
- Work collaboratively within a multidisciplinary team committed to recovery-oriented practice
- Liaise with health, welfare, housing, legal, and community services to support holistic client outcomes
- Support a diverse and often challenging client group, including individuals, families, and communities experiencing alcohol and drug-related harms

Selection Criteria
To help us assess your suitability for the role, please tell us as much as you can about yourself in response to the questions below.
- Degree in health related discipline, which provides eligibility for membership to appropriate professional association or registration with AHPRA for relevant qualification if required
- Demonstrated skills and knowledge in triage, assessment, counselling, providing therapeutic interventions and transfer of care planning for drug and alcohol clients.
- Knowledge of the harm reduction approach to drug and alcohol issues and demonstrated experience in the provision of interventions with drug dependent clients.
- Experience and knowledge in developing person centred, holistic care plans.
- Demonstrated ability to work independently as well as within a multidisciplinary team.
Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
