1. Current Authority to Practice as a Registered Nurse with the Australian Health Practitioners Regulation Agency (AHPRA)  5 years full time equivalent post registration experience Approved post graduate nursing qualifications in a field relevant to the specialty field or such other qualifications or experience deemed appropriate by the organisation.
2. Describe a time when you provided expert clinical advice and developed a care management plan for an older person with complex mental health needs in a community setting. How did you involve carers and other health professionals in the process, and what was the outcome?
3. How have you contributed to the development or delivery of education programs or clinical service planning within a mental health specialty? Please include examples of how you incorporated research findings or upheld professional standards in your practice.
4. Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
