Achieve Something Great
Join the Aboriginal Chronic Care Program (ACCP), a small but dedicated multidisciplinary team committed to improving the health and wellbeing of Aboriginal people across South Western Sydney. The team includes Aboriginal Health Workers and Practitioners, Care Coordinators, Dietitians, Clinical Nurse Consultants, administration staff, a Clinical Team Leader, Facility Manager and Program Manager, working together across clinic sites at Wellama (Bankstown), Budyari (Miller) and the ACCP offices in Miller. Through integrated team care and strong partnerships with GPs, Aboriginal Health Services and other healthcare providers, the program supports early detection, education, lifestyle improvement and coordinated care, with the ultimate goal of empowering clients toward self‑management and contributing to Closing the Gap in health outcomes.
