About the Role
- Lead and deliver culturally appropriate, client-centred care tailored to the unique needs of young Aboriginal people across South Western Sydney.
- Design, deliver and evaluate youth-focused health programs that enhance community engagement – including initiatives like the Aboriginal Health in Schools Program and Love Bites – Cultural Adaptation.
- Collaborate with service partners to ensure care is culturally responsive and reflects the needs of Aboriginal and Torres Strait Islander young people.
- Provide a clinical service component, including cultural strengths mapping, health screening, case management, and preventative care.
- Foster strong, respectful relationships with local Aboriginal communities and internal and external stakeholders to improve service access and trust.
- Hold a leadership role within the Youth Health Service – including supervision of Aboriginal Health Workers, intake and allocation responsibilities, and cultural mentoring.
- Actively contribute to service planning and development through data collection, analysis, and continuous quality improvement initiatives.

Role Details
- Full-time position: Monday to Friday.
- Includes one allocated day off (ADO) per month.
- Based within the Youth Health Service, working across community settings in South Western Sydney
What You'll Be Doing
The Principal Aboriginal Health Worker (PAHW) position is part of the SWSLHD Youth Health Service and is responsible for the provision of culturally safe health services that improve health outcomes for young Aboriginal people in South Western Sydney. The position will develop and lead community development and health promotion initiatives that engage young Aboriginal people. The position will also be responsible for providing individual case management support to Aboriginal clients accessing youth health services and will work collaboratively with the other relevant health professionals to coordinate care and ensure a holistic approach to service provision.
The position will have a lead role in the design, delivery and evaluation of programs for the Youth Health Service, such as Aboriginal Health in Schools Program, Love Bites - Cultural Adaptation. This will include working in partnership with other providers/services to ensure culturally responsive care for Aboriginal and Torres Strait Islander people. The role will have a clinical component related to assessment inclusive of cultural- strengths mapping, screening, case management and preventative care.
The PAHW will direct all aspects of the Youth Health programs including building collaborative partnerships with internal and external health care services across SWSLHD; to increase access to health treatment and prevention services and to improve client’s journeys and outcomes as well as community development and service capacity building
The role has leadership responsibilities including supervision of other Aboriginal Health Workers, intake and allocation, cultural mentoring and active participation in service development activities including collection and analysis of data.
SWSLHD Primary & Community Health provides a comprehensive range of community-based services designed to improve and maintain the health and wellbeing of individuals and communities across South Western Sydney.
These services include prevention, early intervention, assessment, acute and post-acute treatment, health maintenance, and continuing care. The team supports diverse populations, offering specialised programs for children, young people, older adults, and at-risk communities.
With over 800 dedicated staff, including Aboriginal Health Workers, nurses, doctors, and allied health professionals, the service ensures high-quality care close to where people live.

- This is an Identified Aboriginal/Torres Strait Islander Position. Applicants for this position must be of Aboriginal and/or Torres Strait Islander descent through parentage, identify as an Aboriginal person and/or Torres Strait Islander and be accepted in the community as such. Exemption is claimed under Section 14 of the Anti-Discrimination Act 1977 (NSW).
- Relevant tertiary qualification in a health promotion, public health or clinical discipline and/or extensive work related experience.
- Demonstrated application of health promotion principles and skills in planning, implementing and evaluating culturally safe and appropriate health promotion programs and activities (related to improving the health and wellbeing of priority populations and building healthier communities).
- Demonstrated effective organisational skills including managing competing priorities, problem solving and an ability to work as part of a multidisciplinary teams as well as independently to deliver agreed outcomes.
- Demonstrated development of partnerships and relationships across health services/teams focused on quality improvement and capacity building to implement youth related programs.
- Demonstrated ability to play a leadership role within a multidisciplinary team and to supervise staff.
- Ability to undertake health/social needs assessments that demonstrate a comprehensive understanding of Aboriginal culture and social issues in relation to health matters; and make referrals to community service providers.
- Demonstrated excellent verbal and written communication skills, and computer skills with an ability to maintain accurate records in appropriate clinical and or non clinical information systems, collate data to develop project planning and evaluation reports.
- Demonstrated ability to work in partnership to provide optimal health outcomes for Aboriginal communities.
- Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
