South Western Sydney Local Health District (SWSLHD) is seeking a dedicated Clinical Nurse Consultant (CNC) to provide expert clinical advice and leadership within the specialty of Stroke. This pivotal role supports patients, carers, and healthcare professionals by developing, implementing, and evaluating tailored care management plans that improve outcomes for stroke patients. The CNC will work collaboratively across hospitals and multidisciplinary teams to coordinate care and ensure best-practice, evidence-based treatment is delivered. With a strong focus on education, research, and clinical excellence, the CNC also plays a key role in shaping practice standards and driving continuous improvement. What You'll be Doing
The Clinical Nurse Consultant (CNC) is an expert practitioner who, through leadership facilitates evidence based practice. The CNC will promote a service which fosters clinical excellence, research and professional leadership. The CNC contributes to quality health care by collaborating with the health care providers and consumer groups, developing clinical practice standards and evaluating their outcomes. The CNC is responsible for providing services which facilitate effective management of the care in the given specialty.

Liverpool Hospital Eastern Campus is an integral part of the Liverpool Health and Academic Precinct, focusing on education, research, and specialised healthcare services.

- Current Authority to Practice as a Registered Nurse with the Australian Health Practitioners Regulation Agency (AHPRA) and at least 7 years full time equivalent post registration experience, with at least 5 years full time equivalent experience in the specialty field. In addition, the employee must have approved postgraduate nursing/midwifery qualifications relevant to the field in which the employee is appointed, or such other qualifications or experience deemed appropriate by the public hospital or public health organisation.
- SWSLHD has a large intake of stroke patients across the different facilities. Describe key actions in to delivering equitable, safe and high quality care for all of these patients
- Demonstrated evidence in effective communication and collaboration with various clinicians across the multi-disciplinary spectrum and how to foster and maintain these relationships?
- In this role how will you monitor the service and patient outcomes and impact change where necessary? Give some examples of what you will measure against?
- If one site or team is not performing as well as the other sites what are some actions and strategies you would adopt to help improve this?
- Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
