About the Role
Planned Care for Better Health (PCBH) program aims to identify patients at risk of hospitalisation early, strengthen the care provided to them, improve their experience of receiving care and keep them healthier over the longer term.
The PCBH Team Leader is responsible for managing the clinical and operational functions for ACC&R PCBH services along with the Xtend program across the Sydney Local Health District.
This position will be based across Royal Prince Alfred Hospital, Canterbury Hospital and Concord Repatriation General Hospital.

Ideal Candidate
- Appropriate tertiary qualifications or relevant experience in the delivery and management of Aged Care and Rehabilitation clinical services.
- Demonstrated high level interpersonal, verbal, written and electronic communication skills, including a demonstrated capacity of liaise and negotiate with a wide range of stakeholders.
- Current NSW Driver's Licence (P1 and P2 acceptable).
