About the Role
  
Sydney Local Health District’s Community Health Centres have a long and proud history of delivering care in the community including prevention, early intervention, assessment, treatment, health maintenance and continuing care services designed to improve and support the health and wellbeing of individuals and communities.
    
Provide senior clinical leadership within Social Work service provision including optimisation of patient/client care delivery, as well as supporting the mentoring and development of staff including provision of supervision and direction to Level 1/2 staff and students on clinical placements in accordance with the award.
      

The Acute Care Service model of care provides:

  1. Mental Health assessments to people experiencing acute mental health problems ( including a rapid response when indicated) and determining an appropriate pathway to care (eg community treatment; assist consumers access  and admission to inpatient treatment).
  2. Short term intensive treatment in the community often as an alternative to hospital.
  3. In reach to hospital to promote effective follow up post mental health admission.
  4. Developing opportunities for assessment and community treatment at key points (eg; Assessment Unit and Admission Office).
  5. The model provides short term treatment after assessment up until the acute symptoms are resolved or the consumer is effectively transferred and engaged with an appropriate treatment provider.

Ideal Candidate

  • Must hold a Bachelor or post graduate degree in Social Work that provides eligibility for full membership of the Australian Association of Social Workers, with Current NSW Driver’s Licence (P1 and P2 acceptable).
  • Demonstrated advanced clinical knowledge, skill and significant clinical experience in the provision of social work, with experience at Level 2 or higher.
  • Demonstrated high level verbal, written communication and interpersonal skills, including the ability to adapt communication style to suit different audiences.
