About the Role
Sydney Local Health District Virtual Hospital (‘Sydney Virtual’) was the first virtual hospital in Australia. Sydney Virtual supports patient flow and system capacity by shifting care into the community and enhancing the patient experience of care. Services include the Virtual Care Centre, Sydney District Nursing and Integrated Chronic Care services. Sydney Virtual is a District-wide facility with some services also delivered statewide.
The Yudi – 48 Hour Follow Up Care Coordination Service provides culturally safe, coordinated post-discharge support for Aboriginal and/or Torres Strait Islander people living with chronic conditions who have been discharged from an inpatient stay at a Sydney Local Health District hospital. The service aims to improve continuity of care, strengthen care coordination, and support positive health outcomes by ensuring patients receive timely follow-up and access to appropriate health and community services following discharge.
Yudi operates Monday to Friday, 8am to 5pm, excluding Public Holidays.
The program goals are:
- Short term: To follow up Aboriginal patients aged 15 years or older who were admitted to hospital with a chronic disease within two working days of discharge from hospital.
- Medium term: To decrease hospital readmissions within 28 days of discharge.
- Long term: To improve health outcomes for Aboriginal people with chronic disease.

Ideal Candidate
- Must be of Aboriginal descent by proof of Aboriginality, identify as Aboriginal and be accepted as such by the community. Aboriginality is recognised as a genuine occupational qualification and is authorised under section 31 (2h) and 14(d) of the Anti-Discrimination Act 1977.
- Hold or aspire to hold a minimum Certificate III qualification in Aboriginal Primary Health Care.
- Has demonstrated involvement and 'lived experience' with an understanding and commitment to improving health issues in Aboriginal communities.
- Current NSW Drivers Licence (P1 and P2 acceptable).
