About the Role

Sydney Local Health District has a long and proud history of delivering care in the community including prevention, early intervention, assessment, treatment, health maintenance and continuing care services designed to improve and support the health and wellbeing of individuals and communities. 

  

The purpose of the Clinical Nurse Specialist (CNS) position is to provide senior nursing clinical leadership in the development of specialist clinical practice and service delivery within a nursing unit, team or service. The CNS employs advanced clinical skills and exercises extended autonomy of decision making working closely with managers and other senior clinicians. 

  

The Wellbeing Health In-Reach Nurse (WHIN) Coordinator Program in NSW provides school-based health support to children and young people (K–12) and families, with a focus on mental health and social-emotional wellbeing. The role is integral in bridging the gap between education and health services ensuring children, young people, and their families are supported to improve overall wellbeing and engagement across home, school, and community settings. 

  

WHIN Coordinators deliver early intervention, screening and care navigation for students and their families, supporting a range of health needs including mental health, physical health, developmental and psychosocial concerns. This is a dynamic, school-based role requiring strong clinical judgement, effective communication skills, and the ability to collaborate across multiple systems and work autonomously. WHIN Coordinators manage a diverse caseload and respond to complex and evolving student and family needs. 

  

The Clinical Nurse Specialist Grade 2's responsibilities include: 

  • Assess and respond to student health, wellbeing & mental health needs within the school setting, including risk assessment and safety planning. 
  • Identify emerging concerns and provide early intervention strategies. 
  • Coordinate care and facilitate referrals to appropriate health and community services. 
  • Collaborate with school staff, families, and external service providers. 
  • Support marginalised, vulnerable or at-risk students and families by reducing health-related barriers to learning, social and wellbeing needs. 
  • Strengthen partnerships between education and health systems.

Ideal Candidate

  • Current registration with AHPRA as a Registered Nurse (Division 1) with relevant post-registration qualifications and at least 3 years' experience working in the clinical area of their specified post-graduate qualification, or such other qualifications or equivalent experience deemed appropriate by the public hospital or public health organisation. 
  • Ability to prioritise and problem solve to manage the complex clinical care needs of patients through the development and use of clinical management plans. 
  • Highly developed written and verbal communication skills with advanced interpersonal and problem solving skills. 
  • Experience in leading and initiating activities to improve clinical service delivery and patient clinical outcomes.
