The Community Mental Health / Older People’s Mental Health Team model of care states: The “Community Older People’s Mental Health Team” (OPMH Team) is a specialty service of the community mental health system. OPMH Team provide a range of clinical services including intake, assessment, consultation, treatment and care coordination. Care coordination is the main component of this model of care and includes extended care coordination, targeted packages of care and short term care coordination.
Consumers under the care of OPMH Teams will either have:
- Short term interventions and may be discharged or referred back to their GPs or other treatment providers after a period of weeks or months.
- A period of treatment as recommended by evidence based practices.
The clinician will practice in keeping with the standardised Sydney LHD position description for the profession in addition to this position description.
Our unique New Employee Skills & Transition (NEST) Program is designed specifically for community mental health clinicians (CMH) and is a requirement for new clinicians with two or less than two years’ experience working in CMH. During the first three months in the role, you will be supported to participate in the training program to enhance your understanding of the community clinician role and to deliver safe and high-quality care. With guidance from senior clinicians and managers, you'll develop your skills in a structured and supportive environment. The NEST Program complements our other mandatory training requirements and profession-specific development opportunities.

Ideal Candidate
Current nursing registration with the Nursing and Midwifery Board of Australia (AHPRA).
Demonstrated competencies to provide contemporary mental health practice.
Current NSW Driver's Licence (P1 and P2 Licence acceptable).
