What you'll be doing

The Home Therapies Registered Nurse is responsible for delivering safe, high-quality, and patient-centred nursing care to individuals training for and performing dialysis at home, including both Haemodialysis and Peritoneal Dialysis. This role requires the application of advanced nursing skills, clinical expertise, and sound professional judgement to provide thorough assessment, expert clinical care, and ongoing education and support to patients, their families, carers, and the broader multidisciplinary team.

The nurse plays a vital role in empowering patients to manage their treatment independently while ensuring safety, clinical excellence, and continuity of care. Through collaboration, education, and proactive engagement, the Home Therapies RN supports positive patient outcomes and enhances quality of life.

Assess, plan, implement and evaluate nursing care in collaboration with individual/s and the greater health care team to achieve quality patient care outcome.

People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.

Candidates will need to meet the following criteria:

  1. Registered Nurse - Division 1 with Australian Health Practitioner Regulation Agency (AHPRA).
  2. Recent clinical experience and competency in Haemodialysis/Peritoneal Dialysis and willingness to complete a competency training program for managing Home Dialysis.
  3. Demonstrated effective communication using information technology, written, verbal and interpersonal skills with the ability to identify key messages, issues and concerns when communicating with others.
  4. Demonstrated ability to use problem solving skills and a multidisciplinary team approach in the planning, delivery and coordination of patient care.
  5. Demonstrated ability to work collaboratively with colleagues, other health professionals and individuals to provide safe and best practice care.
  6. Ability to provide education and counselling for patients and families potentially requiring Haemodialysis/Peritoneal Dialysis.
  7. A commitment to excellence in nursing practice and continuing professional development.
  8. Valid unrestricted drivers' licence for use in NSW/Australia and willingness to travel in the course of employment.
