This is an exciting opportunity for a motivated social worker to join the Social Work Department at Royal North Shore Hospital. These permanent full-time positions are located within our Chronic and Complex Care Team, working with patients and their families, carers. You will be part of a large, dedicated and highly skilled department working with culturally and socially diverse patients and their families.

What you'll be doing
Joining the Royal North Shore Hospital Social Work Department Chronic and Complex Care team, you will provide a high-quality social work service to a wide range of clinical areas, including but not limited to General Medicine, Neurology, Aged Care, Orthopedics & Respiratory. You will support consumers, carers, and their families, ensuring the provision of optimal health outcomes.

Candidates will need to meet the following criteria:
- Consistently demonstrates behaviours that reinforce the CORE Values of our organisation; Collaboration, Openness, Respect and Empowerment. Demonstrates these behaviours with all stakeholders; colleagues, direct reports, as well as our patients and consumers, and those that care for them.
- Social Work degree and eligibility for the Australian Association of Social Workers (AASW), including the ability to demonstrate a practical application of the AASW Code of Ethics.
- Proven ability to work independently and to establish effectively collaboratively working relationships within a multi-disciplinary team and interagency networks.
- Demonstrated well developed interpersonal, written and verbal communication skills with the ability to communicate with internal and external stakeholders.
- Commitment to participation in professional development and quality improvement activities.
- Demonstrate skills and knowledge in the provision of service to consumers, carers and families requiring assessment, counselling, crisis management, case management, advocacy, intervention, appropriate referral and discharge planning.
- Demonstrated ability to prioritise and manage workload demands, meet deadlines and complete activities (including ability to work within scope of practice and seek assistance when required).
- Demonstrated computer literacy and ability to use health information systems.
