Occupational Therapist, Community Team
We are looking for an experienced OT to join our busy community team. In this role you will use your skills to assess and support patients living at home, with a range of needs. Where you'll be working
Mona Vale Hospital, Community Health Centre
What you'll be doing
Conducting thorough community and home safety assessment for clients in their own homes (predominantly ages 65+)
Identifying risks, providing education and recommendations to clients and their caregivers on risk reduction strategies
Implementing recommendations in line with your clients' goals and risk profile, such as equipment prescription, home modifications, pressure care and falls prevention
Engaging in quality improvement, service and professional development initiatives to ensure delivery of evidence-based care
People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.
- Must hold registration with the Occupational Therapy Board of Australia and an unrestricted NSW Driver's Licence.
- Demonstrate skills and knowledge in the provision of Occupational Therapy service to consumers, carers and families requiring assessment, environmental modifications, home modifications, appropriate referral and discharge planning.
- Commitment to participation in professional development, quality improvement activities and workplace education.
