Occupational Therapist
As an Occupational Therapist Case Manager within the Assertive Outreach Team, you'll work collaboratively with people experiencing complex mental health challenges, providing intensive community-based support that promotes recovery, independence, and meaningful participation in everyday life. Through assertive outreach and person-centred case management, you'll help consumers achieve their goals while remaining connected to their community and support networks.
Employment Type: Temporary Part Time until 30 June 2027 (with possibility of ongoing employment)

Hornsby Ku-ring-Gai Hospital, Derby Road, Gate 6, Building 17, level 2, NSW 2077. The Hornsby Assertive Outreach Team.
What you'll be doing
The Senior Occupational Therapist will be based at Hornsby Ku-ring-Gai Hospital. Specialised units service people with dual diagnosis of mental illness and substance use challenges. The Occupational Therapist will be a champion of the hospital’s commitment to providing recovery-orientated, innovative approaches to supporting our consumers and will bring clinical expertise in the field of mental health rehabilitation. The Occupational Therapist will work collaboratively with consumers, carers, members of the multidisciplinary team and external stakeholders. They will uphold a role in leadership in practice development within the Occupational Therapy department and be committed to quality improvement for OT services and in multidisciplinary activities across the wider hospital.

Candidates will need to meet the following criteria:
- Describe your experience working with people experiencing severe and persistent mental illness in a community mental health setting. How have you supported consumers to achieve recovery goals, increase independence, and enhance participation in everyday life?
- Provide evidence of your qualification and current registration as an Occupational Therapist with the Occupational Therapy Board of Australia (AHPRA). Additionally, outline your relevant clinical experience and demonstrated competencies consistent with an Occupational Therapist Level 3 position, including experience in mental health service delivery, case management, and working autonomously within a multidisciplinary team. A current NSW Driver Licence is required.
Provide an example of your experience managing a complex mental health caseload in a community setting. Describe how you conducted mental health and risk assessments, developed and implemented risk management strategies, and worked collaboratively with consumers, carers, and the multidisciplinary team to coordinate care and achieve positive recovery-oriented outcomes.
