Where you'll be working:
Based at RNSH but working across the Northern Sydney LHD

What you'll be doing
The primary purpose of this position is completing needs assessments of frail older people and their carers in public and private hospitals, in the community and in residential aged care facility (RACF) settings as well as their eligibility for services. This includes providing referrals to services and facilitating access as required by the assessed eligible needs of the patient. A key function of this assessment process is the determination of the client's eligibility for approval to access aged care services as set out by the Aged Care Act. As the role’s purpose is to provide assessment services across the district, some flexibility regarding the location of the role may be required depending on client needs. The role may include requests from the Line Manager to contribute to tasks such as intake, scheduling, triaging, delegating and other demands to support the efficient operation of the service.

Candidates will need to meet the following criteria:
 

 

  1. Proven high standard of written and verbal communication skills. Written skills need to be precise, clinically sound and completed in a timely manner. Proven ability to use Microsoft Office Programs, electronic patient record systems and e-Learning sites.
  2. Relevant tertiary qualifications as per clinical discipline (nursing, occupational therapy, social work, physiotherapy), with full AHPRA registration or eligibility for membership of the relevant Australian peak professional body for the relevant discipline. 
  3. Demonstrated discipline in and clinical experience working in relevant aged care settings and a proven ability to undertake comprehensive assessments, formulate, and implement appropriate care plans for older people.
  4. Have detailed knowledge of My Aged Care, related aged care programs and services with proven ability to use this knowledge to assess and support both routine and complex aged care presentations.
