What you'll be doing
To provide nursing services as a member of a multi-disciplinary community mental health team which offers triage, assessment, treatment, ongoing case management and discharge planning services to individuals in Northern Sydney Local Health Districts.
People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.
Candidates will need to meet the following criteria:
- Current AHPRA registration as a Registered Nurse and current NSW Driver's licence.
- Proven extensive clinical experience in community mental health services and evidence of effective planning, implementation and evaluation, and care coordinating complex mental health consumers.
- Proven experience in evidence of writing NDIS reports, mental health review tribunal reports, effective care planning, implementation and evaluation of consumer goals, and effective liaison with community managed organisations.
