What you'll be doing
The Discharge Patient Flow Concierge Officer- (NSLHD) is part of an improvement team to work on care coordination to improve communication, patient and carer experience and to deliver on patient flow outcomes. It will be a centralised team to work with the sites in a coordinated approach. The roles will assist with earlier and comprehensive discharge planning. This role focuses on supporting the discharge patient flow process while working in collaboration with clinical teams and providing guidance to ensure efficient hospital operations and high-quality patient care.

Candidates will need to meet the following criteria:



  1. Tertiary qualifications in a health-related field and/or equivalent practical experience in improvement projects, together with clinical experience.
  2. Demonstrated experience in the administration of programs in a Healthcare setting.
  3. Demonstrated commitment to customer service and continuous improvement.
  4. Excellent administrative and organisational skills including managing competing priorities in a busy environment
