An exciting temporary part-time (31 hours per week) opportunity exists until 30 June 2026 for an experienced nurse to join the Geriatric Rapid Acute Care Evaluation (GRACE) team at Hornsby Ku-ring-gai Hospital (HKH).

Where you'll be working

Within the Geriatric Rapid Acute Care Evlauation (GRACE) outreach team at Hornsby Ku-ring-gai Hospital

What you'll be doing

The position is required to provide a high-quality outreach service, providing care at home and aiming to avoid hospital admission for people who reside within the community in a private residence or within residential aged care facilities. The position works closely with the multi-disciplinary team comprising of nursing (RN, CNC2, CNS2), medical (Geriatrician, Registrar), Physiotherapist and Administration staff.

People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.

Candidates will need to meet the following criteria:

  1. Current Registered Nurse, registered to practice with AHPRA. Hold relevant post graduate qualifications with at least 3 years’ recent experience in the speciality of Aged Care and /or Community Nursing or such other qualifications or relevant equivalent experience.
  2. Demonstrated ability to work effectively and problem solve both independently and as a member of the multidisciplinary team.
  3. Demonstrated knowledge of and/or experience in initiating and implementing changes to improve clinical service delivery and outcomes through evidence based practice and practice development.
  4. Current unencumbered NSW Driver’s Licence.
