Clinical Nurse Specialist, Geriatric Outreach
An exciting temporary full-time opportunity exists for an experienced nurse to join the Geriatric Rapid Acute Care Evaluation (GRACE) team at Hornsby Ku-ring-gai Hospital.

Remuneration: $2,170.30 - $2,241.60 per week

Where you'll be working:
With the Geriatric Rapid Acute Care Evaluation (GRACE) outreach team at Hornsby Ku-ring-gai Hospital.
What you'll be doing:
- To provide assessment and treatment for older people living in the community who become acutely unwell and are at risk of hospitalisation, as part of the Geriatric Rapid Aged Care Evaluation Team (GRACE)
- To provide practical support for Residential Aged Care Facilities in managing COVID -19 outbreaks.
- To develop education and training regarding COVID -19 outbreaks in Residential Aged Care Facilities.
- Exercise extended autonomy of decision making.
- Exercise professional knowledge and judgement in providing complex care requiring advanced clinical skills.
People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.
Candidates will need to meet the following criteria:
- Current Registered Nurse, registered to practice with AHPRA. Hold relevant post graduate qualifications with at least 3 years’ recent experience in the speciality of Aged Care and /or Community Nursing or such other qualifications or relevant equivalent experience.
- Demonstrated ability to work effectively and problem solve both independently and as a member of the multidisciplinary team.
- Demonstrated knowledge of and/or experience in initiating and implementing changes to improve clinical service delivery and outcomes through evidence based practice and practice development.
- Current unencumbered NSW Driver’s Licence.
