What you'll be doing
The Child & Family Health nurses (CFHN) offer the Sustaining NSW Families (SNF) nurse-led home visiting program for families with moderate levels of vulnerability from mid-pregnancy until their child is two years of age. The SNF Program aims to support healthy parent-child relationship development and support new parents' capacity to optimise their infant's health, well-being, physical, emotional, cognitive, communication, and social development.
The CYF SNF Program is a nurse-led multidisciplinary program offered as part of the Child and Family Health Service within the broader Northern Sydney Local Health District (NSLHD) Child, Youth and Family (CYF) Community Health Service.
The CFH SNF nurses engage with families using a strengths-based approach, supported by a multidisciplinary allied health team. The SNF nurses offer a series of evidence-based, structured interventions that aim to foster positive parent-infant relationships, nurture parent-child attachment and optimise children’s development. The CFHN will complete the advanced practice (SNF) training course (if not prior) within 18 months of starting to support their practice in the Program.
