What you'll be doing
The Aboriginal Registered Nurse is responsible for the provision of high quality, safe and culturally appropriate nursing care within an individualised holistic framework, that is underpinned by compassion and consistent with NSW Health and NSLHD values, policies and standards. In the delivery of nursing care, the Aboriginal Registered Nurse will work under both the direct and indirect supervision of the Executive Director, Aboriginal and Torres Strait Islander Health and the CNC.

The Aboriginal Registered Nurse is responsible for the provision of person centred; evidence-based nursing care that promotes safe, efficient and effectives nursing care and culturally appropriate nursing support to Aboriginal and Torres Strait Islander people with a Chronic Disease. The Aboriginal Registered nurse will work:

• Within a multidisciplinary team within Aboriginal and Torres Strait Islander Health that operates across Northern Sydney Local Health District and will support community based chronic disease services for Aboriginal and Torres Strait Islander clients the districts by facilitating linkages within and between NSLHD, general practitioners (GPs), non-government organisations and Aboriginal Community Controlled Service
• Serve as an advocate for clients and their families/carers, supporting them to make informed decisions, coordinating with services across the Local Health District (LHD) for inpatients, and ensuring any ongoing needs are followed up. This will include but not limited:
- 48 hour follow up program
- Inpatient visits across the LHD. Opportunistic client visits at the Aboriginal Health Service
- Communicate openly and respectfully in line with the CORE values of NSLHD.
• To improve the transition care from hospital to community care by ensuring processes are formalised between Aboriginal specific services across the care continuum
 

People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.

 

Candidates will need to meet the following criteria:

  1. Applicants for this position must be of Aboriginal or Torres Strait Islander, decent, identify as being Aboriginal and be accepted in the community as such. All applicants must supply a letter of Aboriginality signed under the common seal from a recognised incorporated Aboriginal or Torres Strait Islander Community Organisation such as Lan Council or Elders group, Aboriginal Medial Services, or a letter on a letterhead from a Local Aboriginal Land Council demonstrating membership of the Council. Exemption is claimed under Section 14D of the Anti-Discrimination Act 1977 (NSW).
  2. Current Registration with the Nurse and Midwifery Board of Australia.
  3. Good interpersonal skills and ability to communicate with Aboriginal individuals, families and communities, and network with other services and health professionals.
