About the role
Providing leadership and strategic direction regarding the interface between Northern NSW Local Health District (NNSWLHD), National Disability Insurance Scheme (NDIS), and the Aged Care sector.
Implementing strategies which support individualised patient discharge planning, sustainable safe care and improved communication within a multidisciplinary team to ensure sustained patient flow within the LHD. Supporting clinical staff to ensure processes and clear timeframes are in place for discharge planning as patient’s transition through each phase of care creating an overall improved patient experience.
