Social Worker Care Coordinator - Planned Care for Better Health Team

What you will bring to the role

1. Degree in Social Work from an accredited course providing eligibility for membership of the Australian Association of Social Workers and a commitment to maintaining a high level of professional skill and knowledge. Overseas qualifications must supply the AASW Overseas Assessment number.
2. Current Driver's Licence and willingness to use for work purpose travel. This may include driving long distances across NBMLHD.
3. Extensive experience in the delivery of Social Work services, with a demonstrated ability to analyse problems with advanced reasoning skills, set priorities and apply a range of options to support client goal setting and care plans.
4. Demonstrated experience providing Social Work interventions for adult/aged clients who have chronic and complex health conditions in a community setting.
5. Demonstrated ability to work autonomously and collaboratively as a member of a multidisciplinary team to create a holistic and cohesive approach to service delivery, with minimal direct clinical supervision.
6. Demonstrated experience in the provision of clinical supervision for Social Work clinicians and undergraduate students.
7. Demonstrated working knowledge of Integrated Care initiatives / the Strategic Framework for Integrating Care.
8. Demonstrated high level verbal, written and interpersonal skills and the ability to interact constructively and collaboratively with a diverse range of clients and stakeholders throughout all organisational levels.
