Registered Nurse, Team 1 Community Mental Health - ESMHS

Are you a Registered Nurse with mental health experience and a passion for helping others? Wanting to obtain some valuable experience in the community setting or just looking for a change? We would love to receive your application to undertake this role and join our innovative, caring and compassionate multidisciplinary team. 
 

The CMH Clinician will provide comprehensive strengths based and recovery focused assessments with consumers and the formulation of time limited packages of care signed off by consumers and their care givers. These packages will detail the clinical mental health services and other wraparound supports to be provided. They will provide high quality evidence based clinical services including assessment, care co-ordination, intervention, liaison and review as part of a multidisciplinary team for adults referred to the service with a mental illness or disorder.   
  
These interventions will be based on an appropriate model of practice as specified by the individual’s professional background, skills, and experience and will be informed by evidence based treatment guidelines.
