Hawkesbury District Health Service is a teaching hospital in Windsor NSW, providing acute, community and allied health services for the local community and surrounding districts. The service provides 24-hour emergency, medical, surgical, diagnostic, maternity, neonatal, paediatric, palliative, intensive and coronary care services
 

The Discharge Patient Flow Concierge Officer is responsible for supporting administrative processes for discharge planning and assist patients and their families with the process of leaving hospital and transitioning back to their home or other care setting. The position holder will do this by strengthening current discharge planning processes in collaboration with the multidisciplinary team to expedite timely discharge and facilitate integration with out of care hospital and primary care services where appropriate. 
  
You will be responsible for facilitating and enhancing communication between the patient, families, carers, clinical staff, out of hospital care services and primary care to improve the discharge experience for patients, carers and families.

What you will bring to the role

  1. Demonstrated extensive experience in customer service.
  2. Demonstrated high level administrative skills with ability to prioritise and multi-task.
  3. Excellent interpersonal, communication and negotiation skills, in particular the ability to interact effectively with a multidisciplinary team, patients, and carers.
  4. Demonstrated ability to work autonomously, utilise discretion and maintain strict confidentiality whilst working as effective team member.
  5. Adaptable to change with high attention to detail with ability to identify and implement change to facilitate system and process improvements.
  6. Demonstrated ability to problem solve and determine solutions.
  7. Demonstrated high level IT skills and knowledge of Microsoft Office software, including Excel and proficient keyboard skills.
