The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people’s homes, other community venues as well as virtual care (telehealth).

You will work collaboratively in support of healthcare team members, non-government agencies, and acute facilities to provide high-quality care to consumers and their carers, improving outcomes for patients with life-limiting illness.

What you will bring to the role

  1. Current Registered Nurse Division 1 with the Australian Health Regulation Practitioner Regulation Agency (AHPRA) with 5 years full time equivalent postgraduate experience and a minimum of 3 years full time equivalent experience in Supportive and Palliative Care nursing and post graduate qualifications relevant to the specialty area of practice or such other qualifications or experience deemed appropriate by the organisation.
  2. Proven clinical leadership skills and an ability to work collaboratively within a multidisciplinary framework with a demonstrated commitment to excellence in nursing practice
  3. Demonstrated effective communication and inter-personal skills across a variety of mediums and an understanding of the practice development framework
  4. Proven ability to implement changes and develop direction in clinical practice by the utilisation of evidence based research
  5. Demonstrated skills in strategic and clinical service planning including an understanding of organisational behaviour, change management and cultural change processes
  6. Demonstrated skills in clinical teaching, competency in the use of contemporary information systems and experience in the development of specialty education programs
  7. Demonstrated clinical expertise in client-centred consultancy with the ability to work independently
  8. Current valid NSW drivers licence with proven good driving record and willingness to use for work purposes. This will include driving long distances across the Local Health District
