The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people’s homes, other community venues as well as virtual care (telehealth).

 

Lawson Community Health Centre is seeking a compassionate Registered Nurse to join our Community Palliative Care Team in a Monday to Friday role. Working under the Clinical Nurse Unit Manager, you will deliver high quality, safe and effective nursing care to clients across the Local Health District.

In this role, you will provide person-centred care using a flexible, responsive, wellness and reablement approach aligned with Australian Aged Care Quality and National Standards. You will undertake assessment, planning, implementation and evaluation of care based on evidence-based practice, working collaboratively with clients, families, GPs, specialists and community partners.

You will also provide support, advice and education to Community Nurses, Allied Health professionals, clients and carers, contribute to relevant quality and standards working groups, and assist service delivery across sites as required. The role includes supervising Enrolled Nurses and delegating clinical care within their scope of practice.

What you will bring to the role

  1. Current registration to practice as a Registered Nurse (Division 1) with the Australian Health Practitioner Regulation Authority (AHPRA) with demonstrated experience and clinical skill in nursing as a Registered Nurse including completion of a new graduate Registered Nurse program or equivalent experience working as a Registered Nurse
  2. Demonstrated skills and experience working in the provision of palliative care to clients who have a life limiting illness
  3. Demonstrated capacity to provide effective nursing care that is person centred, family inclusive and culturally sensitive to people accessing health services and an ability to liaise with other health professionals to facilitate case management and care planning.
  4. Demonstrated ability to organise and prioritise work and an ability to effectively work both within a team and autonomously.
  5. Demonstrated effective communication skills (written and verbal) and interpersonal skills.
  6. Demonstrated commitment to professional development, evidence-based practice and an awareness of changes within the health care environment.
  7. Demonstrated knowledge of and ability to utilise information technology systems within a variety of applications (e.g. Cerner) and commitment to undertake training in electronic health record systems as required.
  8. Current valid NSW State driver's licence (including P1 or above) and willingness to use for work purposes. This may include driving long distances across the LHD.
