About the Role
We are seeking an experienced and motivated Rural Generalist Nurse Practitioner – Aged Care to join our integrated care team, based in Port Macquarie.
This is a unique opportunity to work in a progressive, future-focused service delivering high-quality, person-centred care to older people across the Mid North Coast. The Rural Generalist Nurse Practitioner (NP) Aged Care role delivers complex, person centred aged care across multiple service touchpoints, including Aged Care Outreach, Hospital in the Home, MNCLHD Virtual Care, and other acute, aged, chronic, and complex care settings. The NP will provide care across diverse services and modalities both face-to-face and virtual ensuring timely, appropriate interventions are delivered in the most suitable setting across MNCLHD.
Grounded in nursing values, knowledge, and practice, the Nurse Practitioner role offers innovative, flexible healthcare that complements and enhances multidisciplinary care. The NP’s scope of practice is defined by the clinical context in which they are authorised, enabling responsive, high quality care tailored to the needs of older people in rural and regional communities
This is a Full Time Permanent position based onsite in Port Macquarie.
About You
You are a passionate and highly skilled Nurse Practitioner who is motivated by delivering meaningful care to older people and shaping contemporary models of care.
You will bring:
- Current registration with the Nursing and Midwifery Board of Australia as a Nurse Practitioner, or you are actively working toward NP endorsement with the capacity to meet endorsement requirements within two years
- Demonstrated advanced practice expertise in Aged Care, with the ability to critically evaluate, translate, and apply contemporary research to inform complex clinical decision making and care interventions.
- Excellent leadership and interpersonal skills, with a proven ability to educate, mentor, and support others through effective teaching and learning strategies within both the specialty and the broader nursing profession
- Ability to clearly articulate the clinical reasoning underpinning diagnostic, prescribing, and treatment decisions, ensuring care is evidence-based, person-centred, and grounded within a nursing framework.
- Demonstrated high level of capability in performing advanced procedures, treatments, and interventions informed by specialist knowledge and best practice
- Strong clinical reasoning skills, with the ability to clearly articulate diagnostic, prescribing, and treatment decisions that are evidence-based and person-centred
- Well-developed leadership and communication skills, with experience in education, mentoring, and supporting others
- A commitment to ongoing professional development, maintaining currency of practice, and contributing to service improvement and innovation
A current NSW driver’s licence and the ability to travel as required are essential.
