What you'll be doing
The Discharge Planning Coordinator works in collaboration with the health care team to coordinate and facilitate transfer of care and discharge planning processes for hospital inpatients.   

The Discharge Planning Coordinator practices in accordance with the Nursing and Midwifery Board of Australia Standards for Practice for Registered Nurses, and the policies and procedures of NSW Health and Mid North Coast Local Health. District 

About you
You are a registered nurse with relevant post-registration qualifications and at least three years’ experience in your specialty (or equivalent). You work effectively both independently and as part of a multidisciplinary team, with experience across inpatient and community settings, including My Aged Care, ACAT, NCAT, NDIS, and GP services.

You have a solid understanding of patient flow, discharge planning, and resource utilisation, and apply strong problem-solving skills to support coordinated, patient-centred care. You communicate clearly and professionally across all formats and are committed to evidence-based practice and continuous improvement.
