A Discharge Planner plays a crucial role in facilitating the safe and efficient transition of patients from hospital to their homes or other care settings. By coordinating care, offering education, and connecting patients with vital resources, discharge planners ensure smooth transitions that contribute to patients' overall well-being and long-term health outcomes. They act as essential liaisons between healthcare providers, patients, and their support networks, ensuring that comprehensive discharge plans are created and implemented. Discharge Planners significantly influence key performance indicators related to hospital length of stay, readmissions, and patients who discharge against medical advice, all of which are integral to the overall performance of healthcare facilities. These professionals contribute to the optimization of transfer of care and hospital access targets, helping ensure that patients are transitioned out of hospital care appropriately. 
  
In this role, the Discharge Planner will also play a pivotal part in supporting the safe and culturally responsive transition of Aboriginal patients back into their communities. By providing clinical support, comprehensive follow-up care, and connecting patients with the most appropriate referral pathways for out-of-hospital care, this position ensures Aboriginal patients receive treatment options tailored to their cultural needs. This approach helps reduce hospital readmissions and empowers Aboriginal patients to thrive within their communities, ultimately improving their health outcomes.
